No.52 Toward the construction of Inagi City's version of the "Regional Comprehensive Care System"

In response to the rapid aging of the population, we will provide medical care, nursing care, preventive care, housing, and lifestyle support in an integrated manner so that even if you become in need of care, you can continue to live your own life in your familiar home or community. We are proceeding with the construction of a "community comprehensive care system" that will be implemented in the future.
Regarding the promotion of home medical care/nursing care cooperation, the city has already started a project to promote feeding and swallowing function support since FY2013, a project to promote home medical care/nursing care cooperation since FY2014, and in May 2015. A consultation room has been opened at the Inagi City Medical Association.
Currently, we are working on "multidisciplinary collaboration" as a pioneering example. Kashiwa City is attracting attention as an advanced example, and with the support of Fujitsu Research Institute and the Center for Home Health Care at the University of Tokyo School of Medicine, Kashiwa City is also implementing a program based on Kashiwa City's program that also takes Inagi's actual situation into consideration.
Another pioneering example is the early implementation of the "Nursing Care Prevention and Daily Life Support Comprehensive Project." This will transfer preventive care services and lifestyle support services for those requiring support, which were previously provided as insurance benefits under the long-term care insurance system, to municipal projects.We will provide a variety of services in nearby municipalities while paying close attention to individual needs. It's something we're trying to deal with. Since April 2015, only Inagi City and Kunitachi City have started this program in the Tama area of ​​Tokyo.
The first theme for future initiatives is "dementia countermeasures." We currently have dementia support coordinators at two regional comprehensive support centers in the city, and we plan to increase the number in the future.
The training of dementia supporters has been ongoing since 2008, and after 100 lectures, approximately 3,500 supporters have been produced. In addition, starting in 2015, there is a plan to train all city employees over a three-year period, and I also took the course and became a Dementia Supporter.
The Dementia Care Path, which shows the flow of appropriate service provision according to the state of dementia, is scheduled to be completed as a pamphlet in FY2016.
We plan to establish an intensive initial dementia support team in fiscal 2017, centered around Inagidai Hospital, which was designated as a community-based dementia care center by the Tokyo Metropolitan Government in September last year.
The second theme is the formulation of the ``Inagi City Medical Plan''. This is separate from the national and metropolitan medical plans and is unique to the city.
It is estimated that the number of elderly people and patients with dementia will double by 2025, and building a community-based comprehensive care system will not be achieved just by enhancing nursing care and nursing care prevention, but will require close cooperation with medical care. is essential. While assessing the current state of medical resources, understanding home medical care needs, and promoting collaboration between medical care and nursing care, how should we develop the infrastructure of medical resources to meet the anticipated increase in medical needs in the future? We would like to develop a better plan by thoroughly exchanging opinions with doctors from the medical association.
In the future, we will work together with citizens and other stakeholders to build a community comprehensive care system as early as possible, and aim to create a community where people can live a healthy and happy life in a familiar community without relying on residential facilities as much as possible.
