For enrollment procedures, please visit the City Hall or the Branch Office (Hirao, Wakabadai).
If enrollment procedures are required
For those who are not enrolled in the company's health insurance, if you meet any of the following conditions
- When moving in from other cities or towns
- When I quit my job
- When you are no longer covered by social insurance dependents
- When a child is born
- When a person receiving Public Assistance has their Public Assistance discontinued
Items Required for Procedures
- Certificate of Loss of Social Insurance Eligibility (in cases 2 and 3 above)
- Identification documents (driver's license, passport, My Number Card, etc.)
- Identity verification documents / Individual number (My Number) verification documents
- Notification in such cases
