When you stop your workplace health insurance and enroll in the National Health Insurance

If you are enrolled in workplace health insurance (such as social insurance, National Health Insurance Association, or mutual aid insurance) and you withdraw from that insurance without enrolling in another health insurance, you must enroll in the National Health Insurance.
Please complete this procedure within 14 days after withdrawing from your workplace health insurance.
If the enrollment procedure is delayed, you will be required to pay the insurance tax retroactively, so please be careful.

Required Items for Procedures

  1. Documents that clearly state the date of loss of eligibility for workplace health insurance
    Example) Certificate of Loss of Eligibility for Social Insurance
    Note: Documents that only state the resignation date (such as separation notices) cannot be processed.
  2. Identity Verification Documents
  3. Personal Number (My Number) Confirmation Document

About National Health Insurance Tax

The National Health Insurance Tax is levied from the month in which you acquire eligibility for the National Health Insurance (i.e., the month you withdraw from your workplace health insurance).
Please note that it is not levied from the month you complete the procedures.

When you join the workplace health insurance

If you join a workplace health insurance plan, you need to go through the procedure to withdraw from the National Health Insurance.
This must be done by the individual joining, not by the company or other entity.

Required Items for Procedures

  1. Information about the newly joined health insurance
    All members, including dependents, are required.
    You can confirm using one of the following methods.
    • Notification of Qualification Information
    • Qualification Confirmation Document
    • Valid workplace Health Insurance Card
    • Confirmation via My Number Portal
  2. Health Insurance Card or Qualification Confirmation Document for National Health Insurance
    All members who withdraw from the National Health Insurance must return their cards.
  3. Identity Verification Documents
  4. Personal Number (My Number) Confirmation Document

Regarding Medical Consultations After Loss of Qualification

If you use the Health Insurance Card for the National Health Insurance after the qualification acquisition date of your workplace health insurance, you may be required to refund 70% to 90% of the medical expenses (insurer's share).
If you have used it by mistake, please contact the medical institution you visited as soon as possible.

When a child is born

When a subscriber of the National Health Insurance gives birth, a one-time payment of 500,000 yen for childbirth and childcare will be provided.
Note: For births until March 31, 2023, the amount will be 420,000 yen.

When a member of the National Health Insurance dies

When a subscriber of the National Health Insurance passes away, a funeral expense of 50,000 yen will be provided.

When you lose your Health Insurance Card or qualification confirmation document, or when they are damaged

Please bring your identification documents and proceed with the reissuance procedure.
After December 2, 2024, depending on whether you have registered to use the My Number Card as a Health Insurance Card, you will receive a qualification confirmation document or a notification of qualification information.

Note: On holidays, open hours are from 8:30 AM to 11:45 AM and 1:00 PM to 5:00 PM
Attention: Holiday openings are only at City Hall.
