If you have lost your Specific Health Checkup voucher, please bring your Health Insurance Card and a photo ID (either a driver's license, My Number Card, passport, resident basic registry card, or Physical Disability Certificate) to the Insurance and Pension Division. Alternatively, you can call us, and we can send it to you by mail.
