Cancellation of Application for My Number Card (Individual Number Card)
If you wish to cancel your application for the My Number Card (Individual Number Card) for any reason after applying, the following procedures are required.

- Legal representative (only when the person and the legal representative are in the same household and have a parent-child relationship)
Note: In the case of a voluntary agent, cancellation of the application cannot be done by phone.

Please let us know that you would like to cancel your My Number Card application.
We will conduct a hearing (to verify your identity, etc.).

Application for Legal Representative
In addition to the above, the following documents are required.
- Identification documents of the agent A1 point or B2 points (see below for "List of Identification Documents")
- Documents certifying agency rights (such as family register copies)
Note: In the following cases, 2 is not necessary.
- If a parent and child reside in Inagi City, have the same household on the Certificate of Residence, and the parent-child relationship can be confirmed by the details on the Certificate of Residence
- If your registered domicile is within Inagi City
Application for Voluntary Agent
In addition to the above, the following documents are required.
- Identification documents of the agent A1 point or B2 points (see below for "List of Identification Documents")
- Power of Attorney
List of Identity Verification Documents
- Identification documents must be valid and within their expiration date.
- Identification documents must match the Certificate of Residence with either "name and date of birth" or "name and address".

- Physical Disability Certificate
- Driving History Certificate (limited to those issued on or after April 1, 2012)
- Residence Card
- Special Permanent Resident Certificate, etc.

- Health Insurance Card
- Long-Term Care Insurance Card
- Pension Book
- Public Assistance Recipient Certificate
- Infant Medical Certificate
- Maternal and Child Health Handbook (for those who have received a certificate of birth registration)
- Student ID
- Employee ID (Student ID and Employee ID must include either "Name and Date of Birth" or "Name and Address") etc.
Points to Note
- For those who have applied for the "reissue of the My Number Card" and have already paid the fee, please note that refunds are not possible.
- If you wish to obtain the My Number Card again, you will need to submit a new application for the issuance of the My Number Card. For more details, please contact us.
