Express Issuance Application (My Number Card Issuance Application)
The My Number Card usually takes about one month from application to receipt, but starting December 2, 2024, it will be possible to issue the My Number for infants and those who have lost their card in about one week.
(1) Apply for the card at City Hall or a Branch Office → (2) Receive the card at home (registered residence) etc.
Children aged 0 can apply at the same time as the birth registration. For more details, please check the link below.
In the case of express issuance, a reissuance fee due to loss or other reasons will be 2,000 yen (if no electronic certificate is included, it will be 1,800 yen).
Note: The normal reissuance fee is 1,000 yen (if no electronic certificate is included, it will be 800 yen).

Requirements for Special Issuance
The issuance of express tickets is applicable to those who meet the criteria in the table below.

Period from application to receipt
It takes about 1 week from the application.
However, if any of the following apply, it may take more than 1 week for the card to arrive.
- For those with external characters in their name or address, and where automatic character conversion is not possible
- In case the express issuance application is crowded nationwide
- In case of holidays and the year-end and New Year period
- For children aged 0 years old who are not registered as residents in Inagi City
Application Method [For First-Time Card Applicants]
Please bring the necessary documents to the Citizen Affairs Division, Hirao and Wakabadai Branch Offices, and apply in person.
Required Documents
- Identification documents A2 points or A1 point + B1 point or S1 point + B2 points (see below for "List of Identification Documents")
- Notification Card (for those who have one)
- Basic Resident Registration Card (for those who have one)
Note: The facial photo will be taken using a dedicated terminal at the Citizen Affairs Division (for individuals aged 1 and older). Those bringing a facial photo should prepare one that is "taken within the last 6 months, with no background and no hat (L size vertical recommended)." Additionally, for infants under 1 year old, the photo for the My Number Card is not required, so it does not need to be brought.
If the person is under 15 years old or is an adult under guardianship, the following documents are also required.
Additionally, both the person and the legal representative must come to the office.

- If the person is under 15 years old, a copy of the family register (not required if the person and their legal representative are in the same household, and the person's registered domicile is within Inagi City)

Note: There are no open hours on holidays.
How to Receive
It will be sent directly from the national agency to the address registered in your residence via "express and simple registered mail."
You will receive the envelope containing your My Number Card in person from the postal delivery staff.
Notes
- Please be aware that photos taken with dedicated terminals have lower image quality than those taken at photo studios, as they are of a resolution equivalent to that of a smartphone.
- We cannot provide the photos taken.
- The fee for the facial photograph is free.
- The application review will be conducted by a national agency. If there are any issues with the facial photo or other documents, we will contact you by phone or other means. In that case, you will need to visit the office again.
- The PIN for the My Number Card will be filled out in the Application Form.
- Those who incur an issuance fee will need to pay the fee at the time of card application. Please note that the fee is non-refundable after the application is accepted.
- Please be sure to receive your My Number Card within the storage period at the post office.
- In the event that you are unable to receive the card within the postal service's holding period, it will be returned to City Hall. To collect it, you will need to visit the main City Hall. It will be held for a certain period, but after the holding period has expired, it will be discarded. Please note that there will be a fee for obtaining the card afterwards.
