Issuance in-person method (My Number Card application)

The method of receiving the card in person involves applying for the My Number Card from home using a smartphone or by mail, and then visiting City Hall to collect the card.

This is a method available for those who do not have a government-issued photo ID to obtain a My Number Card.

Application Method

Please apply in person using one of the following methods.

Application using a smartphone

Please scan the QR code listed on the "Application Form for Issuance of Personal Number Card" or "Personal Number Notification" and apply on the website for issuance. If you do not have the application form, please refer to the method of obtaining the application form at the bottom of the page.

Application using the photo booth in the town

Select "Application for Individual Number Card" from the touch panel of the available photo booth, insert the money for taking the photo, and hold the QR code of the "Application Form for Individual Number Card" up to the barcode reader.
Follow the instructions on the screen, enter the required information, take a facial photo, and submit it.

How to Obtain the Application Form (For Those Who Do Not Have the Application Form)

The ID-required application form needed for applications via computers, smartphones, or photo booths in the city can be obtained by the following methods.

Request at the City Hall

The individual or a member of the same household should bring identification documents (driver's license, Health Insurance Card, etc.) and make the request.

Online Billing

Will be sent by regular mail that does not require forwarding to the registered residence.

How to Receive

The My Number Card must be received in principle by the individual themselves.
Note: If the individual is under 15 years old or a person under guardianship, the presence of a legal representative and two other individuals is required.

About Receiving by Proxy

• You can delegate the receipt of the card to an agent only if you are unable to come to the office due to unavoidable reasons such as hospitalization or physical disabilities. Please check the following link for the requirements for agent receipt.
• Please understand that work or being busy does not constitute an unavoidable reason.
• When receiving on behalf of someone, in addition to proof of hospitalization or proof of admission to a facility that certifies the inability to come to the office, identification documents with photos of both the individual and the representative are required. The required documents differ from the usual ones, so please consult us in advance if you have any questions.
