We will assist you with the online application for the My Number Card (Application Support)
Staff will take the necessary photo for the application of the My Number Card (Individual Number Card) using a dedicated terminal in the Citizen Affairs Division and assist with the online application.
Since the staff will operate the terminal, please feel free to use this service if you are unsure about the application process.
Required Documents
(1) Identification Documents
- Identification documents are limited to originals that are within the validity period.
- Identification documents must match the "name and date of birth" or "name and address" on the Certificate of Residence.

- My Number Card (with photo)

- Physical Disability Certificate, etc.
- Driving History Certificate (limited to those issued on or after April 1, 2012)
- Residence Card
- Special Permanent Resident Certificate
- Health Insurance Card
- Long-Term Care Insurance Card
- Pension Book
- Basic Pension Number Notification
- Various Pension Certificates
- Public Assistance Recipient Certificate
- Various Medical Certificates (Maru Nyuu, Maru Ko, Maru Ao)
- Maternal and Child Health Handbook (for those who have received a certificate of birth registration)
- Student ID (one that includes "name and date of birth" or "name and address")
- Employee ID (one that includes "name and date of birth" or "name and address")
(2) Application Form for Issuance of Personal Number Card and Electronic Certificate (My Number Card Application Form)
Note: You can proceed even without an Application Form.

Please bring the necessary documents mentioned above and come directly to the City Hall Citizen Affairs Division or Branch Office.
No reservation is necessary.

Notes
- Please be aware that photos taken with dedicated terminals have lower image quality than those taken at photo studios, as they are of a resolution equivalent to that of a smartphone.
- We cannot provide the photos taken.
- The fee for the facial photograph is free.
- It takes about one month (approximately) from application to the issuance of the My Number Card.
