When the pension to be paid to the deceased remains, the unpaid pension will be paid to the surviving family members who shared their livelihood.
Notification window
If the recipient of the pension dies, please submit the claim form for the unpaid pension along with the notification of death of the pension recipient to the Fuchu Pension Office or the Pension Section of the City Hall. increase).
Note: If you are able to claim the Survivor's Welfare Pension, please do so at the Fuchu Pension Office.
Note: If the deceased was a member of a mutual aid association, it may be necessary to submit to each mutual aid association.
Things necessary
If the claimant is from the same household as the deceased
(1) Certified copy of family register ・If the claimant and the deceased are on the same family register ⇒The certified copy of the claimant's family register (issued after the date of death and shows the relationship to the deceased)
・If the claimant is on a separate family register from the deceased person ⇒ A combination of a copy of the claimant's family register and a copy of the deceased's removed family register, etc.
(2) Resident's card of the claimant (post date of death, details of the claimant and all members of the household, including "permanent domicile", "household head", "relationship", and "changes" thing)
(3) Removed resident card of the deceased (post date of death, registered domicile and relationship)
(4) Claimant's deposit (savings) passbook (5) Pension certificate, pension certificate, basic pension number notice, pension book of the deceased (6) Claimant's identity verification documents (My number card, driver's license, etc.) )
Note: Submission of (2) and (3) can be omitted by filling in the claimant's main number.
