Please tell me the procedure if I lose my pension certificate.

If you lose your pension certificate, apply for a reissuance at the Fuchu Pension Office.
It can also be reissued by mailing an application for reissuance of an annuity certificate. Forms are also available at the Health Insurance and Pension Section of the city hall.
What to bring
Something that shows your basic pension number/pension code and identity verification documents (my number card, driver's license, etc.)
In the case of a representative, a power of attorney and identification documents for the representative (My Number Card, driver's license, etc.)
