What to bring

Identification document (My Number card, driver's license, etc.)
Note: You can also apply through the City Hall Insurance and Pension Division, but it will take about a month and a half for the document to be mailed to your home. If you are in a hurry, please apply directly at the Fuchu Pension Office.

What you need when applying by proxy

In addition to the above (things to bring): (1) Power of attorney (2) Agent identity verification documents (My Number card, driver's license, etc.)

Category 2 insured person or Category 3 insured person

Please apply through your employer or your spouse's employer.
