Please tell me about the reissuance (lost) of the basic pension number notification.

The procedure differs depending on the category of the insured person.

What to bring

Identity verification documents (My number card, driver's license, etc.)
Note: You can also apply at the Health Insurance and Pension Section of the city hall, but in that case it will take about a month and a half for it to be mailed to your home. If you are in a hurry, please apply directly at the Fuchu Pension Office.

Necessary items for proxy application

In addition to the above (things to bring), (1) Power of Attorney (2) Identity verification documents (My Number card, driver's license, etc.)

Category 2 insured person or Category 3 insured person

Please apply through your employer or your spouse's employer.
