If your name has changed due to marriage, etc., you will need to complete the name change procedure at the Citizens Affairs Division. No procedures are required at the Health Insurance and Pension Section.
Note: Please notify the place of work or the place of work of the spouse of the name change of the second and third insured persons respectively.

It is necessary to submit a “Notification of Change of Name of Pension Recipient”.
Notification forms are available at the Pension Office and the Health Insurance and Pension Section of the city hall.
A new pension certificate after the name change will be issued, so please attach all your pension certificates and submit them.
Also, please go through the name change procedures at the receiving financial institution of the pension.
Note: If you are receiving a Survivor's Pension and your name has changed because of marriage or adoption, you will no longer be entitled to a Survivor's Pension. Please check with the pension office for details.
