QuestionPlease tell me the process from applying for long-term care certification to using services.

Response

(1) Apply for Long-Term Care Insurance nursing care or support

  • Apply at the Long-Term Care Certification Section of Inagi City Hall (2nd floor, window 4).
  • Apply by mail (to the Long-Term Care Certification Division, Inagi City Hall).
  • Request to act on behalf of applications to the Community General Support Center and home care support offices.

(2) Receive a certification survey (home visit survey)

Investigators will visit your home, hospital, or facility and conduct an assessment based on a nationwide standardized questionnaire regarding your physical and mental condition.

(3) Submit the 'Questionnaire for the Preparation of the Attending Physician's Opinion Statement' to the medical institution designated at the time of application.

The creation of the attending physician's opinion statement is directly requested from the city to the medical institution. If the questionnaire is not submitted to the medical institution, the physician may not be able to create the attending physician's opinion statement.

(4) Primary Judgment

Based on the results of the visit survey, a judgment will be made using a computer.

(5) Secondary Judgment

In the Long-Term Care Certification Review Committee, we will review and determine how much care is needed based on the first judgment results, the content of the certification investigation, and the opinion statement from the attending physician.

(6) Notification of Certification Results

The certification result notification and the Long-Term Care Insurance Health Insurance Card will arrive approximately 30 days after the application. There may also be cases where you are certified as not applicable (independent).

(7) Creation of Care Plan

Not applicable (independent) individuals

Those who answer the basic checklist and show a decline in living functions will consult with the Community General Support Center to determine the services to be used based on their situation.

Persons requiring support level 1 and 2

The creation of a long-term care prevention service plan (care plan) will be discussed by the individual, family, and Community General Support Center to determine the services to be used.

Persons requiring care level 1 to care level 5

We will contract with home services and home care support offices, including community-based services, and request the creation of a care plan.

  • Care managers will work with the individual, family, and service providing Business Operators to develop a care plan. They will select facility services and the desired Long-Term Care Insurance facilities, and contract directly.
  • The facility's care manager will create a care plan suitable for the individual.
