QuestionPlease tell me the procedures when a pension recipient passes away.
Response
When there is a Pension remaining to be paid to the deceased, the unpaid Pension will be paid to the surviving family members who were living together.
Notification Window
If a person receiving a pension passes away, please submit the Notification of Death of Pension Recipient along with the Application for Unpaid Pension to the Fuchu Pension Office or the Pension Section at City Hall (the application forms are available at the counter).
- Note: If you are eligible to apply for the Survivor's Welfare Pension, the procedure will be at the Fuchu Pension Office.
- Note: If the deceased was a member of a mutual aid association, it may be necessary to submit documents to each mutual aid association.
Necessary Items
If the applicant is in the same household as the deceased
- Family Register
- If the applicant and the deceased are in the same family register
Applicant's family register copy (after the date of death, showing the relationship with the deceased) - If the applicant is on a different family register from the deceased
Combination of the "applicant's family register copy" and the "deceased's family register removal copy" (must be after the date of death and show the relationship between the two)
- If the applicant and the deceased are in the same family register
- Certificate of Residence of the requester
(Must be issued after the date of death, containing information for the requester and all household members, and must include "registered domicile," "head of household," "relationship," and "changes") - Certificate of Residence for Deceased
(Issued after the date of death, showing the registered domicile and relationship) - Depositor's Savings Passbook
- Pension certificate, gratuity certificate, basic pension number notification, pension book of the deceased
- Identity verification documents of the applicant (My Number Card, driver's license, etc.)
Note: By entering the applicant's My Number, you can omit the submission of items 2 and 3.
If the applicant is from a different household than the deceased
- Family Register

- Spouse
- Child
- Parents
- Grandchild
- Grandparents
- Siblings
- Relatives within three degrees of kinship other than these individuals
When proceeding with the application, identification documents of the requester (My Number Card, driver's license, etc.) are required. In the case of a representative, a power of attorney and identification that can confirm the representative's identity are necessary.
