Identity verification documents (My Number Card, driver's license, etc.)
Note: You can also apply through the Insurance and Pension Division at City Hall, but it will take approximately one and a half months to be mailed to your home. If you are in a hurry, please apply directly at the Fuchu Pension Office.

Required items for agent application

In addition to the above (items to bring)

  1. Power of Attorney
  2. Identification documents for the agent (My Number Card, driver's license, etc.)

Insured Person Type 2 or Insured Person Type 3
