If you lose your Pension certificate, you will need to apply for a reissue at the Fuchu Pension Office.
You can also request a reissue by mailing the Pension Certificate Reissue Application Form. The form is also available at the City Hall Insurance and Pension Division.

Items to Bring

Documents that show the basic pension number and pension code along with identification documents (My Number Card, driver's license, etc.)
In the case of a representative, a power of attorney and identification documents of the representative (My Number Card, driver's license, etc.)
