Please tell me how to apply for long-term care and support certification under long-term care insurance.

Applications are accepted at the Elderly Welfare Division Nursing Care Certification Section or at the window of each Regional Comprehensive Support Center.

Things necessary

Over 65 years old

(1) Long-term care insurance card (2) Document showing the name of the attending physician, name of the medical institution, name of the department, address, and phone number (notes, etc. are acceptable)

40 to 64 years old

Note: You must have health insurance.

(1) Health insurance card (2) The name of the attending physician who is familiar with the mental and physical condition of the person suffering from the specified disease (one of the 16 specified diseases), the name of the medical institution, Documents that show the department name, address, and phone number (notes, etc. are acceptable)
