What should I do if I do not have an attending physician when I apply for certification for long-term care or support required under long-term care insurance?

It is desirable to appoint a doctor who was in charge of examining the injury or illness that triggered the application for long-term care or support requirement certification, or a doctor who regularly visits the hospital and understands the situation of the person's daily life well. If you do not have a doctor, choose a new primary care doctor and visit a medical institution.
When you visit a doctor, please fill out the "Questionnaire for creating a statement of your doctor's opinion" and bring it with you, and hand it directly to the doctor.
At that time, let them know that you are applying for certification for long-term care or support.
