Please tell me the flow from applying for certification of need for long-term care or support to using the service.

(1) Apply for long-term care insurance or support required

  • Apply at the Nursing Care Certification Section at Inagi City Hall (Counter 4 on the 2nd floor).
  • Apply by mail (addressed to the Nursing Care Certification Section at Inagi City Hall).
  • Ask the regional comprehensive support center or in-home nursing care support office to act on your behalf.

(2) Take a certification survey (visit survey)

An investigator visits the patient's home, hospital, and place of admission, and investigates the mental and physical conditions of the person based on a nationwide questionnaire.

(3) Submit a "questionnaire for preparing a statement of opinion of the attending physician" to the medical institution specified at the time of application.

The city will directly request the medical institution to prepare the chief physician's statement. If the questionnaire is not submitted to the medical institution, the doctor may not be able to create a doctor's opinion.
Note: (2) and (3) are in no particular order

(4) Primary judgment

Judgment will be made by computer based on the results of the visit survey.

(5) Secondary judgment

Based on the results of the primary judgment, the contents of the certification survey, and the doctor's opinion, the nursing care certification examination committee will examine and judge how much nursing care is necessary.

(6) Notification of certification results

About 30 days after the application, you will receive a certification result notification and a long-term care insurance card. It may be certified as non-applicable (independent).

(7) Creation of a care plan

Non-applicable (independent)

After answering the basic checklist, those who are experiencing deterioration in daily life functions will consult with the Regional Comprehensive Support Center and decide which service to use according to their situation.

What does the Regional Comprehensive Support Center do?

Long-term care level 1 to long-term care level 5

At-home services including community-based services ・Contract with an in-home care support office and request the creation of a care plan. 

  • A care manager will discuss with the patient, family members, and service providers to create a care plan. Choose the facility services and long-term care insurance facilities you want and make a contract directly.
  • The facility care manager will create a care plan that is right for you.
