What does a care support specialist (care manager) do?

A specialist with a wide range of knowledge about long-term care, we create a long-term care service plan (care plan) according to the physical and mental conditions of the user and his or her family. They also play a role of coordinator between the user and the nursing care service provider, such as coordinating with the nursing care service provider and managing insurance benefits according to usage records.
