What should I do if I can't find my nursing care insurance certificate or certificate of coverage share?

You can have your card reissued by applying for a reissue. You can apply by mail or at the Inagi City Hall Elderly Welfare Division (window number 4 on the second floor). If you apply by mail, it will be sent by regular mail.
The required documents vary depending on who is applying (coming to the counter), so please check.

• Those who are currently applying for long-term care (support required) certification or who will apply for long-term care (support required) certification in the future do not need to have their long-term care insurance certificate reissued. When we send authorization result, we send identification of new nursing-care insurance person insured. Please wait until the certification result comes out.
