Foster parents and adopted children (if the adopted child is under 15 years old, the legal representative of the adopted child)
Necessary Items for Notification
- Adoption Registration Form (Two witnesses required for adults)
- Identification documents of the person who came for the Notification
- My Number Card (For those who have the card and wish to change the details. It also serves as a form of identification.)
- the Basic Resident Registration Card (for those who have the card and need to change the registered information. It also serves as a form of identification.)
- When adopting a minor, a permit from the family court is required (however, it is not necessary when adopting one's own or spouse's direct descendants)
- If a foster parent with a spouse is adopting a child alone, the consent of the spouse is required.
Available Procedure Dates
Open on weekdays from 8:30 AM to 5:00 PM
- Note: The office is open on holidays from 8:30 AM to 11:45 AM and from 1:00 PM to 5:00 PM
- Outside of the above hours, applications will be accepted at the City Hall duty room.
- On holidays and when accepted at the City Hall's night duty room, only "custody" will be available.
- Note: Notifications related to family registers are not accepted at the Branch Office.
