Foster parents and adopted children (if the adopted child is under 15 years old, the legal representative of the adopted child)

Necessary Items for Notification

1. Adoption Registration Form (Two witnesses required for adults)
2. Identification documents of the person who came for the Notification
3. My Number Card (For those who have the card and wish to change the details. It also serves as a form of identification.)
4. the Basic Resident Registration Card (for those who have the card and need to change the registered information. It also serves as a form of identification.)
5. When adopting a minor, a permit from the family court is required (however, it is not necessary when adopting one's own or spouse's direct descendants)
6. If a foster parent with a spouse is adopting a child alone, the consent of the spouse is required.

Available Procedure Dates

Open on weekdays from 8:30 AM to 5:00 PM

• Note: The office is open on holidays from 8:30 AM to 11:45 AM and from 1:00 PM to 5:00 PM
  • Outside of the above hours, applications will be accepted at the City Hall duty room.
  • On holidays and when accepted at the City Hall's night duty room, only "custody" will be available.
• Note: Notifications related to family registers are not accepted at the Branch Office.
