Notice: In principle, parents will be the notifier. If someone else is to be the notifier, please contact the family register section in advance.

Necessary Items for Notification

1. Birth Registration Form (Certification by a doctor or midwife is required)
2. Maternal and Child Health Handbook

Notice: Foreign nationals may need additional documents, so please contact the family registry section in advance.

Procedures Related to Birth

The main procedures related to childbirth are as follows

• Childbirth and Childcare Lump-Sum Payment

  Joining the National Health Insurance

Available Procedure Dates

Open on weekdays from 8:30 AM to 5:00 PM

• Note: The office is open on holidays from 8:30 AM to 11:45 AM and from 1:00 PM to 5:00 PM
  • Outside of the above hours, applications will be accepted at the City Hall duty room.
  • On holidays and when accepted at the City Hall's night duty room, only "custody" will be available.
• Note: Notifications related to family registers are not accepted at the Branch Office.
