Identity Verification for Various Notifications and Requests

When making notifications such as moving in or out, or requesting a copy of the Certificate of Residence or a full certificate of family register, it is necessary to present identification documents of the person making the notification or request.
This is implemented to prevent false notifications or requests by third parties and to protect personal information.

Please note that the identification documents required for the application and issuance of the My Number Card are different, so please check here.

Examples and Categories of Identity Verification Documents

Documents with a photo attached issued by government agencies… (A)

Driver's License, Driving History Certificate, Passport, My Number Card, Basic Resident Registration Card (B Card with Photo), Residence Card, Special Permanent Resident Certificate, Temporary Refugee Permit, Provisional Stay Permit, Physical Disability Certificate, Mental Disability Health and Welfare Certificate, Seaman's Book, Marine Engineering License, Hunting Gun and Air Gun Possession Permit, War Injury Certificate, Real Estate Transaction Specialist Certificate, Electrician License, Certified Electric Work Practitioner Certificate, Special Electric Work Qualification Certificate, Airworthiness Inspector Certificate, Aviation Personnel Skill Certificate, Flight Operations Manager Skill Examination Pass Certificate, Training Qualification Certificate, Power Vehicle Operator's License, Examination Pass Certificate (Security Guard), Radio Operator License, Rehabilitation Handbook, Small Vessel Operator's License, Identification Card for Employees of Public Offices Including Independent Administrative Agencies and Special Corporations or Equivalent Documents

Documents issued by public offices that do not have the individual's photo attached... (B)

Health Insurance Card, various Pension certificates (passbooks), Long-Term Care Insurance card, Public Assistance recipient certificate, Basic Resident Register card (A card without a photo) or equivalent documents

Documents proving that you are the person other than A and B... (C)

Cash cards, credit cards, medical examination tickets, savings passbooks, employee IDs, student IDs, various membership cards, or documents equivalent to these.

  • Note 1 The identification documents you need to present may vary depending on the procedure. (See the next section and below)
  • Note 2 Items with an expiration date are only valid within the validity period.
  • Note 3 The My Number notification card cannot be used as a form of identification.

When requesting a certificate

Request for the issuance of family register certificates (family register copies), etc.
(Except in the case of wide-area issuance)

  • 1 point for B + 1 point for C (limited to those with a face photo)
    or
  • 2 points for B
Any of the above, and one that proves either "name and address" or "name and date of birth"
A copy of the identification document that you attach when requesting by mail must include your address.

Request for Issuance of Family Register Certificate
(In case of wide-area issuance)

When making a Notification or application

Notifications and Applications Documents to be submitted Notes
Resident relocation notifications such as moving-in notifications, moving-out notifications, and head of household change notifications
  • 1 point for A
    or
  • 2 points from B and C
If you are a foreign national and are moving in or relocating, in addition to personal identification, you will need the residence cards or special permanent resident certificates of all individuals subject to the move.
Marriage registration, mutual divorce registration, adoption registration, mutual adoption termination registration, acknowledgment registration, non-acceptance application
  • 1 point for A
    or
  • 1 point for B + 1 point for C (limited to those with a face photo)
    or
  • 2 points for B
Any of the above, and one that proves either "name and address" or "name and date of birth"
 
Application for Various Special Permanent Resident Certificates
  • 1 point for A
    or
  • 2 points from B and C
In addition to identity verification, a Special Permanent Resident Certificate is required for most applications.
Application for Seal Registration 1 point for A  
Notification of the abolition of seal registration, loss of seal registration certificate, etc.
  • 1 point for A
    or
  • 2 points from B and C
 
Old Name Addition (Certificate of Residence, My Number Card, etc.)
  • 1 point for A
    or
  • 2 points from B and C
In addition to identity verification, a family register copy, etc. that includes the former name (all family register copies, etc. from the one that includes the former name to the one that includes the current name) will be required.
