Necessary Items for Notification

• Divorce application form (Two adult witnesses are required for mutual consent divorce)
• Identification documents of the person who came to file the Notification for a mutual divorce
• My Number Card (For those who have the card and wish to change the details. It also serves as a form of identification.)
• the Basic Resident Registration Card (for those who have the card and need to change the registered information. It also serves as a form of identification.)
• In the case of a mediated divorce, a copy of the mediation record is required.
• In the case of a divorce by judgment, a copy of the judgment document and a certificate of finality are required.
• In the case of a divorce by reconciliation, a certified copy of the reconciliation document
• Copy of the acknowledgment document in case of an acknowledged divorce
• In the case of divorce by judgment, a copy of the judgment and a certificate of finality are required.

Available Procedure Dates

Open on weekdays from 8:30 AM to 5:00 PM

• Note: The office is open on holidays from 8:30 AM to 11:45 AM and from 1:00 PM to 5:00 PM
  • Outside of the above hours, applications will be accepted at the City Hall duty room.
  • On holidays and when accepted at the City Hall's night duty room, only "custody" will be available.
• Note: Notifications related to family registers are not accepted at the Branch Office.
