If there are any changes to your name, address, etc., you will need to change the design of your My Number Card and continue using it.
Please complete the procedure at your city hall or branch office.
Note: This process takes time. If you submit your application near the end of business hours, you may not be able to complete the process on the same day. Please submit your application with plenty of time to spare.

Required documents
The person himself/herself or a member of the same household
(1) My Number Card (verify the four-digit PIN you set when you received your card)
(2) Identification document of the representative (driver's license, passport, etc.)
important point
- If you are unable to enter your 4-digit PIN, you will need to reset your PIN. For more information, please see here .
- When processing procedures for members of the same household, please bring the My Number cards of all household members and, once each member's 4-digit PIN number can be verified, one household member can process all of the procedures at once ( please confirm the 4-digit PIN number with each person in advance ).
- The electronic signature certificate installed on your My Number card will become invalid if your address or name changes. If you wish to have a new one issued, you will need to come to the office in person to complete the procedure.
- However, if the procedure is done on the same day as the moving-in/moving-out procedure, and if the necessary items are brought with them, a member of the same household may complete the procedure on your behalf. Voluntary agent other than the principal or a member of the same household
(1) My number card of the principal (2) Identity verification documents of the agent (driver's license, passport, etc.)
(3) Power of Attorney (4) A sealed envelope with the personal identification number of the person in question written on it
How to create "(4) A sealed envelope with the personal number card PIN number written on it"
- The person fills out the "personal identification number (4 digits) of my number card" and "the person's name" on paper such as letter paper.
- Put the paper in an envelope and seal it with glue.
- Hand the envelope to the representative and bring it in a state where the PIN cannot be seen.
important point
- A staff member will open the envelope and enter the PIN number on your behalf to complete the procedure.
- If you bring the envelope in an unsealed or opened state, we will not be able to process your application.
- If the PIN is incorrect, the procedure cannot be completed.
- If you wish to issue a digital certificate for signing, the procedure cannot be completed on the same day.
Those who issued an electronic certificate for signature
The electronic signature certificate will automatically become invalid if your name, address, etc. changes due to a transfer or family register notification.
If you wish to have a new card issued, please bring your My Number card with you.

Issuing electronic signature certificates for members of the same household (simultaneous with moving in or moving house)
A member of the same household may complete the procedure on your behalf, provided that they complete the procedure on the same day as your moving-in/moving-out procedure and bring the necessary items with them.
If the procedure cannot be completed on the same day as the move, or procedures other than the move (such as a name change due to marriage), members of the same household cannot complete the procedure.

(3) A power of attorney. (4) A sealed envelope containing the PIN number (6 to 16 alphanumeric characters) for the person's electronic signature certificate.
How to create "(4) A sealed envelope containing the PIN number of the person's digital signature certificate"
- The person writes down the "PIN number for the person's electronic signature certificate (6 to 16 alphanumeric characters)" and "the person's name" on a piece of paper such as a letter paper.
- Put the paper in an envelope and seal it with glue.
- Hand the envelope to the representative and bring it in a state where the PIN cannot be seen.
important point
- If you are processing an application for multiple people, you will need “(3) a power of attorney” and “(4) an envelope containing the PIN code for the signature electronic certificate” for each person .
- You will also need a 4-digit PIN. Please confirm this with the person in advance.
