Changes and continued use of My Number Card (personal number card) (change of address, etc.)

Updated: July 2, 2024

  • When there is a change in your name, address, etc.
  • When you move to Japan from overseas and continue to use your overseas card in Japan

You will need to change the face of your My Number Card and continue using it.
Please complete the procedure at your city hall or branch office.
Note: This process takes time. If you submit your application near the end of business hours, you may not be able to complete the process on the same day. Please submit your application with plenty of time to spare.
  

Continuous use
  • Change of address due to moving from outside Inagi City (transfer)
  • Change of address due to moving from overseas and changing to a domestic card (moving overseas)

 
Note: If any of the following apply to you, you will not be able to continue using your My Number Card.
When obtaining a new My Number Card, a reissue fee will be charged, so please apply as soon as possible.
・If you have not filed your moving-in notification within 14 days of starting to live in Inagi City. ・If 30 days have passed since the date you planned to move out. ・If 90 days have passed since you filed your moving-in notification.

Ticket change
  • Change of address due to moving within Inagi City
  • Name change due to notification of marriage, divorce, adoption, etc.
  • Former family name (maiden name) included
  • Change of address due to change of town name and lot number

     

    Required documents

    When the person or a member of the same household visits the office

    (1) My Number Card (verify the four-digit PIN you set when you received your card)
    (2) Identification documents for visitors (driver's license, passport, etc.) Note: Not required if the visitor is in person.

    important point

    • When processing procedures for members of the same household, please bring the My Number cards of all household members and, once each member's 4-digit PIN can be verified, one household member can process all of the procedures at once ( please confirm the 4-digit PIN number with each member in advance ).
    • If you are unable to enter your 4-digit PIN, you will need to reset your PIN. For more information, please see here .
    • The electronic signature certificate installed on your My Number card will become invalid if your address or name changes. If you wish to have a new one issued, you will need to come to the office in person to complete the procedure.
    • However, if the procedure is done on the same day as the moving-in/moving-out procedure, and if the necessary items are brought with them, a member of the same household may complete the procedure on your behalf.     When a representative (someone other than the person himself/herself or a member of the same household) visits the office

    (1) Your My Number card (2) Your representative's identification document (driver's license, passport, etc.)
    (3) Sealed power of attorney [Procedure for changing address and name on My Number card, for separate households] (PDF: 264KB)

    important point

    • Please place the power of attorney in an envelope and seal it with glue or other means so that the representative cannot see the PIN number (the procedure cannot be completed if the envelope is brought to you unsealed or opened).
    • A staff member will open the envelope and enter the PIN number on your behalf to complete the procedure.
    • If the PIN is incorrect, the procedure cannot be completed.
    • If you wish to issue a digital certificate for signing, the procedure cannot be completed on the same day.
    • If you are having an agent submit a change of address notification or relocation notification, you will need to prepare both a "Power of attorney for change of address notification (relocation notification)" and a "Power of attorney [for My Number card address/name change procedure/separate household]".

       

    Those who have issued digital certificates for signatures

    The electronic signature certificate will automatically become invalid if your name, address, etc. changes due to a transfer or family register notification.
    If you wish to have a new card issued, please bring your My Number card with you.
    In addition, you will need to verify your My Number card PIN (two types: 4 digits and electronic signature certificate) . For details, please see here .

    Issuing electronic signature certificates for members of the same household (simultaneous with moving in or moving house)

    A member of the same household may complete the procedure on your behalf, provided that they complete the procedure on the same day as your moving-in/moving-out procedure and bring the necessary items with them.
    If the procedure cannot be completed on the same day as the move, or procedures other than the move (such as a name change due to marriage), members of the same household cannot complete the procedure.
    (1) Your My Number card (2) Your representative's identification document (driver's license, passport, etc.)
    (3) Sealed power of attorney [My Number Card address change procedure, same day notification, for the same household] (PDF: 289KB)

    important point

    • If you are processing for multiple people, you will need a "(3 ) Power of Attorney [For the same household, for My Number Card address change procedures and notifications submitted on the same day] " for each person .
