[Reception by proxy] Receiving your My Number card (visiting the office at the time of issue)

In principle, the person must collect the My Number Card in person. If you are collecting it in person, please see here .
You may appoint a representative to collect your card only if you are unable to come to the office due to unavoidable reasons such as hospitalization or a physical disability.
When collecting the item on behalf of another person, you will need to provide proof of hospitalization or admission to a facility, or other proof that you cannot come to the office, as well as photo identification of both the person and the representative.
The required documents are different from usual, so if you have any questions, please contact us in advance.
Please see below for the requirements for proxy receipt and the necessary documents.

 

Requirements for proxy collection and certificates that cannot be received in person

 

Reasons for not being able to visit the officeCertificate of inability to visit the office (any one of the following)

Student ID card (digital student ID cards are not accepted), student registration certificate

Institutional residents

Admission certificate, photo certificate (issued by the facility director) (PDF: 155KB)

Those who have been hospitalized for a long time

A medical certificate, hospitalization medical plan, and receipts verifying hospitalization (issued within the last month),

Pregnant womenMaternal and Child Health Handbook, receipt or medical examination ticket showing that you had a prenatal checkup
Those studying abroadA copy of your visa (where the visa can be confirmed) and a copy of your student ID card from your study abroad destination

People on long-term business trips (such as crew members on long-term voyages)

Documents issued by your employer showing that you are on a long-term business trip or long-term voyage (documents that are deemed to make it difficult for you to come to the office in light of objective circumstances such as your job content, place of work, and working conditions)

People who avoid social participation and remain at home for long periods of time (people who are withdrawn from society)

Photo ID (issued by the public support organization you are consulting with and the head of said organization) (PDF: 211KB)

Note: The photo ID can be used in place of identification or proof that you cannot visit the office for those certified as requiring nursing care or nursing support, those residing in facilities, those hospitalized for a long period of time, and those who are withdrawn from society. Please download and create the form.
Note: Please understand that work-related reasons, being busy, etc. are not unavoidable reasons.
 

How to receive

1. Once the card has arrived at city hall and is ready to be given to you, the city will send you a "Notice of My Number Card Collection."
2. After reading the notice, please gather the necessary documents and make a reservation for pickup online or by phone.

3. On the scheduled date and time, have your representative come to the special My Number counter on the first floor of City Hall with all the necessary documents.
Please see the notification sent by the city for more details.
Note: If the documents you bring are incomplete or missing, you will not be able to receive your My Number Card.
 

Required documents [For those getting a card for the first time]

(1) Notification of issuance of the Individual Number Card and response form (white postcard sent by the city)
(2) Two pieces of identification documents A or one piece of identification document A and one piece of identification document B or three pieces of identification document B (one piece of identification document has a photograph) (See "List of identification documents" below)
(3) Two pieces of identity verification documents of the representative or one piece of identity verification document of the representative (see "List of identity verification documents" below)
(4) Certificates that cannot be obtained by visiting the office (see "Requirements for receiving by a proxy and certificates that cannot be obtained by visiting the office" above)
(5) Notification card (if you have one)
(6) Basic Resident Registration Card (if you have one)
 
If you are under 15 years of age or an adult ward, you will need the following documents:
(7) [If the person is under 15 years old] A copy of the family register ( not required if the person and the legal representative are from the same household or if the person's registered domicile is in Inagi City)
(8) [If the person is an adult ward] Certificate of registered matters
 

Required documents [For renewal/reissue]

(1) Notification of issuance of the Individual Number Card and response form (white postcard sent by the city)

If you are under 15 years of age or an adult ward, you will need the following documents:
In addition, both the person and their legal representative must be present at the office.

Free <br id="3"/>However, in the following cases, a fee of 1,000 yen (800 yen if no electronic certificate is installed) will be charged.

  • Reissuance of My Number Card due to loss, damage, defacement, or fire
  • Reissuance of card due to failure to change address within the designated period
  • Re-issuance of My Number Card after surrendering it for personal reasons
  • Re-issuance of a residence certificate after it has been cancelled due to non-residency or other reasons
  • [Foreigners] Reissuance of My Number cards due to expiration of mid- to long-term residents (excluding those with a status of residence of Highly Skilled Professional No. 2 or permanent resident)

Note: Screenshots of digital student ID cards and digital employee ID cards are not permitted. You will need to be present to launch and operate the app.
Note: A photo ID can be used in place of identification for those under 18 years of age, adult wards, certified care recipients, nursing care support recipients, facility residents, long-term hospitalized patients, and recluses. It can also be used as proof that you cannot visit the office (except for high school and technical college students). Please download and create the form from the link above.

Notes

  • When issuing your My Number card, we will check your face against the photo on the card. Please note that you will be asked to remove any masks, etc., when checking your face.
  • Notification cards and Basic Resident Register cards will be collected at the time of issuance.
  • It cannot be issued at the Hirao or Wakabadai branch offices.
  • You will set your own PIN using the touch panel at the counter.

Regarding the period from application to receipt

It takes approximately one month from application to receipt of your My Number Card.
If you have not received a notification within two months of applying, please contact us and we will check the issuance status for you.
