Presentation method at the time of issuance (Application for issuance of my number card)

In the "Visit-to-Issuance" method, when applying for a My Number Card, you apply from home using your smartphone or by mail, and when the card is issued, you come to the city hall to receive it.
(1) Apply for the card at home, etc. → (2) Receive the card at city hall. Even if you do not have a government-issued identity verification document with a photo, you can obtain a My Number card.

How to apply
Please apply in person using one of the following methods:
- Application using smartphone
Scan the QR code printed on the "Individual Number Card Application Form" or "Individual Number Notification" and apply on the application website. If you do not have the application form, please see here .

How to apply by mail
- Online application (next office)
The Citizens Division Citizen Counter, Hirao Branch Office, and Wakabadai Branch Office can assist you with online applications.
Reception hours are different from usual, so please check the following for details.
We will help you apply online for your My Number Card (Application Assistance)
- Application using ID photo machine in town
Select "My Number Card Application" from the touch panel of the photo ID machine, insert money for the photo, and hold the QR code on the "My Number Card Application Form" over the barcode reader.
Follow the instructions on the screen, enter the required information, take a photo of your face, and submit it.

How to obtain the application form (if you don't have the application form)
The application form with ID required for applying via computer, smartphone, or camera in town can be obtained in the following ways.
- Request in person
The person in question or a member of the same household should bring identification (driver's license, health insurance card, etc.) to make the request.
- Claim online
It will be sent to your registered address by ordinary mail without the need for forwarding .

How to receive
In principle, the person must collect the My Number Card in person.
Note: If the person is under 15 years of age or an adult ward, a legal representative and two other people must come to the office.
About proxy collection
- Only if you are unable to come to the office due to unavoidable reasons such as hospitalization or physical disability, you can entrust a representative to collect your card. Please see the following link for the requirements for proxy collection.
- Please understand that work or being busy is not an unavoidable reason.
- When collecting on behalf of another person, proof of hospitalization or admission to a facility, proof that you cannot come to the office, and photo identification documents for the person and the representative are required. The required documents are different from usual, so please contact us in advance if you are unsure.
