Regarding cancellation of application for issuance of My Number Card (Individual Number Card)

If you have applied for a My Number Card (individual number card) and wish to cancel your application for any reason, please follow the steps below.

Apply by phone

How to apply

Applicant

  • The person himself
  • Legal representative (only if the person and the legal representative are from the same household and have a parent-child relationship)

Note: If you are a voluntary representative, you cannot cancel your application over the phone.

Please tell us that you would like to cancel your My Number Card application .
We will conduct a hearing (hearing to confirm your identity, etc.).

Required documents

Application by the person

(1) Identity verification documents A1 point or B2 points (see "List of identity verification documents" below)

Legal representative application

In addition to the above, the following documents are required:
(1) Agent's identity verification documents A1 or B2 (see "List of identity verification documents" below)
(2) Documents proving authority of representation (copy of family register, etc.)
Note: (2) is not required in the following cases:
・If the parent and child live in Inagi City, and the resident record indicates they belong to the same household, and the parent-child relationship can be confirmed by the relationship on the resident record. ・If the legal address is in Inagi City.

Voluntary representative application

In addition to the above, the following documents are required:
(1) Agent's identity verification documents A1 or B2 (see "List of identity verification documents" below)
(2) Power of attorney
 

List of identity verification documents

  • Identity verification documents must be within the validity period.
  • Identity verification documents are limited to those whose ``name and date of birth'' or ``name and address'' match the resident record.

Driver's license my number card (with face photo)
Basic resident register card (with face photo)
Passport Physical disability certificate Driving history certificate (limited to those issued after April 1, 2012)
Residence card, special permanent resident card, etc.

B

Health insurance card Nursing insurance card Pension book Public assistance recipient card Infant medical card Maternal and child health handbook (with proof of birth registration)
Student ID, employee ID (student ID and employee ID must have your name and date of birth or name and address written on them), etc.


important point

  • Please note that if you have applied for a reissue of your My Number Card and have already paid the fee, it will not be refunded.
  • If you wish to obtain a My Number Card again, you will need to apply for a new My Number Card. Please contact us for more information.
