Cancellation of Issuance Application for My Number Card (Individual Number Card)

If you wish to cancel the issuance application for some reason after applying for a My Number Card (Individual Number Card), the following procedures are required.

  • Legal representative (only if the principal and the legal representative are in the same household and have a parent-child relationship)

Note: In the case of a voluntary agent, the application cannot be canceled over the phone.

Please tell them that you want to cancel the application for My Number Card .
Hearings (interviews for identity verification, etc.) will be conducted.

Apply in person

(1) Identity verification documents A1 or B2 (see “List of identity verification documents” below)

Legal representative application

In addition to the above, the following documents are required:
(1) Agent's identity verification documents A1 or B2 (see "List of Identity Verification Documents" below)
(2) Documents certifying authority of representation (family register, etc.)
Note: (2) is not required if:
・When parents and children living in Inagi City are in the same household on the resident card and the parent-child relationship can be confirmed by the relationship on the resident card ・When the permanent domicile is in Inagi City

Voluntary proxy application

In addition to the above, the following documents are required:
(1) Agent's identity verification documents A1 or B2 (see "List of identity verification documents" below)
(2) Power of attorney

  • Identity verification documents must be valid.
  • Identity verification documents are limited to those whose "name and date of birth" or "name and address" match the resident card.

Driver's license My number card (with face photo)
Basic Resident Register card (with face photo)
Passport Identification booklet for the physically disabled Driving career certificate (limited to those issued after April 1, 2012)
Residence card, special permanent resident card, etc.

Health insurance card Long-term care insurance card Pension book Social protection recipient certificate Baby medical certificate
Student ID card Employee ID card (Student ID card and employee ID card must have "name and date of birth" or "name and address" written on them), etc.


important point

  • Please note that if you have applied for reissuance of your My Number Card and have already paid the fee, you will not be able to receive a refund.
  • If you wish to obtain a My Number Card again, you will need to apply for issuance of a My Number Card again. Please contact us for more information.
