We will help you apply for your My Number Card online (Application Assistance)

We have started a service to help you apply for your My Number Card (Individual Number Card) online by taking a photo with a dedicated terminal.
This is a method to apply and receive your My Number Card at the city hall (you will need to visit the city office twice).
The staff will operate the terminal when applying online, so please feel free to use it if you do not know how to apply.
Flow of application procedure
Take a photo of your face using the dedicated terminal at the Citizens Affairs Division and apply for a My Number Card.
1. Please bring the required documents listed below to reception .
・City Hall Citizens Division Counter (weekdays only)
・Hirao branch office, Wakabadai branch office (every Tuesday and Thursday only)
Note: Reservations are required by the day before.
2. Taking a face photo (staff assistance)
Take a photo of your face with a special device.
The face photo fee is free.
3． Online application (staff assistance)
Apply directly on a dedicated terminal using the photograph of your face.
After applying, it will take approximately one and a half months (estimate) for your My Number card to be issued.
We will send you a notice of issuance, so you will need to come to Inagi City Hall Citizens Division to pick up your card.
Required documents
(1) Identity verification documents
- Please bring any of the following documents.
- Identity verification documents must be valid.
| 1point | Basic Resident Registration Card (with face photo), driver's license, driving history certificate (limited to those issued on or after April 1, 2012), passport, physical disability certificate, residence card, special Permanent resident card, etc. |

| 2 points | Health insurance card, long-term care insurance card, various pension certificates, public assistance certificate, etc., infant medical care card, student ID card, employee ID card (student ID card and employee ID card are limited to those with name and date of birth), etc. |
(2) Personal number card issuance application form and electronic certificate issuance application form (My number card application form)
Note: You can complete the procedure even if you do not have the application form.
Notes
- Please note that the quality of photos taken with a dedicated device is inferior to that of photos taken at a photo studio (the resolution is the same as that of a smartphone).
- We cannot give you the photos you have taken.
- The staff will operate the dedicated terminal.
