We will help you apply online for your My Number Card (Application Assistance)

Staff will take the photograph required for your My Number Card (Individual Number Card) application using a dedicated terminal in the Citizens' Affairs Division and assist you with the online application.
Staff will operate the terminals, so please feel free to use them if you are unsure of how to apply.
Required documents
(1) Identity verification documents
- Identification documents must be original and valid .
- Identification documents must match the name and date of birth or name and address on your resident registration card.

(2) Application form for issuance of My Number Card and electronic certificate (My Number Card application form)
Note: You can still apply even if you do not have the application form.
How to apply
Please bring the above required documents to the Citizens' Affairs Division of City Hall or a branch office in person.
No reservations are required.

[Reception hours] Weekdays 8:30am to 4pm (excluding noon to 1pm)
Note: Not available on holiday opening days.
Hirao Branch
[Reception hours] Weekdays 9:00 am to 4:00 pm (excluding 12:00 pm to 1:00 pm)
Wakabadai Branch
[Reception hours] Weekdays 9:00 am to 4:00 pm (excluding 12:00 pm to 1:00 pm)
Notes
- Please note that the quality of photos taken with the dedicated device will be inferior to photos taken at a photo studio, etc. (They have the same resolution as a smartphone).
- Photos taken cannot be handed over.
- The photo is free of charge.
- After application, it will take approximately one month (estimated) for your My Number Card to be issued.
