About writing the old name (maiden name) together

A copy of your resident card, my number card, and your maiden name on your seal registration certificate, etc.

Due to the enforcement of the Cabinet Order to partially revise the Basic Resident Register Law Enforcement Order, etc., from November 5, 2019, the old copy of the resident card, my number card, seal registration certificate, etc. It is now possible to write surnames (family register transcripts, family register extracts, removed family register copies, past family register surnames listed in removed register extracts) at the same time.

[Documents that can be written with the former name]

  • Copy of resident record
  • Resident record certificate
  • Seal registration certificate (You can also register your seal with your former name.)
  • My Number Card
  • Electronic certificate for signature of public personal identification service

Note: If you register your former name, your former name will be listed in all of the above [documents that can be written with your former name]. (It is not possible to display either the former name or the current name on the family register.)
Note: Foreign nationals cannot list their former surnames.

Former Mr. who can be written together

register

  • Only one former name per person can be listed.
  • If you are writing your old name together for the first time, you can arbitrarily choose from the past names listed in your family register, etc.
  • The former name once entered will continue to be entered even if the name is changed due to marriage, etc.
  • Even if you move to another municipality, your old name will continue to be listed.

change

  • If the surname is changed, only the last surname can be changed.

delete

  • It is possible to delete the old Mr. However, if the former name is deleted, only when the name is changed, one of the former names newly created after the deletion can be selected and written together.

Applicant

  • The person or a member of the same household
  • An agent with a power of attorney

Note: The Power of Attorney must be signed by the authorizing person.

Items required for application

  • A certified copy of the family register, etc., in which the former surname is stated (all family registers, etc., from those in which the former surname is stated to those in which the current surname is stated)
  • Documents verifying the applicant's identity
  • My Number Card

Note: A certified copy of a family register, etc. means a certified copy of a family register, a copy of a family register, a copy of a removed family register, or a copy of a removed family register.

Notes

Since it is necessary to attach a family register, etc., to the application, it is not possible to apply for an old name registration or change on the day of family register notification such as marriage.
Note: For details on the number of days from the date of notification of the family register to the time it is recorded in the family register and the family register can be obtained, please contact the person in charge of the family register of the municipality where the notification was made.
