- Notice: Please be aware that the fees for issuance by mail request and at the City Hall counter are different.
- Notice: Requests for the mailing of the seal registration certificate will not be accepted.
- Notice: For wide-area issuance, mail requests are not accepted.
- Notice: The identification codes for providing electronic family register certificates and electronic removal certificates will be free of charge if (1) requested simultaneously with the same content family register certificate or removal certificate, or (2) requested via My Number Portal.
Billing Method
Please enclose the following four items and send them to the Citizen Affairs Division.
- Mail Request Form (Please download the Application Form from the pages for "Certificate of Residence Forms" and "Family Register Certificate Forms")
- Fees and fixed amount postal money orders (Please purchase the required amount at the post office without receiving change)
- Reply envelope (with stamp affixed and address written)
- A copy of identification documents (such as driver's license, Health Insurance Card, etc.)
- Note 1 In the case of a mail request by the applicant, it will be sent to the registered residence address as a general rule.
- Note 2 When requesting a family register certificate, a copy of an identification document that includes your address is required.
- Note 3 Due to the revised law, the request for notification of the insurer number and the insured person's symbols and numbers has been restricted. Therefore, when taking a copy of the Health Insurance Card, please ensure that the insurer number and the insured person's symbols and numbers are not visible in the copy.

About Fixed-Amount Postal Money Orders
- Please send a fixed amount postal money order equivalent to the fee so that no change is given.
- The validity period is 6 months from the date of issue. For administrative reasons, we kindly ask you to send a fixed amount postal money order that has 10 days or more remaining validity.
- Please send the fixed amount postal money order with both sides unfilled.
