Those who own a My Number Card
- The My Number Card is limited to those with a valid electronic signature certificate. When applying, it is necessary to enter the electronic signature certificate PIN (6 to 16 alphanumeric characters).
- It is necessary to enter the secret code for ticket information input assistance (a 4-digit number).
- If you have not updated the information on your My Number Card during past relocations, you will not be able to use the online application. Please update your My Number Card at the reception window of your local city or town. (In the case of Inagi City, you can complete the procedures at the Citizen Affairs Division, Hirao Branch Office, or Wakabadai Branch Office.)
- Please note that applications from third parties are not allowed.

Application Method
- Install the dedicated app (Electronic Authentication My Number Sign) on your smartphone in advance.
- Access the online application form and register your email address.
- A login URL will be sent to the registered email address.
- Log in to the application form again from the URL and fill in the required information.
- After confirming the input details, you will perform an electronic signature. Read the My Number Card using a dedicated app on your smartphone and press the application button.
- After confirming the application details, payment will be made on the payment screen.
- After reviewing the application details and confirming the payment information at the Citizen Affairs Division, the certificate will be mailed.

This is a dedicated app for reading signature electronic certificates. It is used during the online application process for the certificate.
Notice: Only compatible smartphones with My Number Card are allowed.
Notice: To use the online application, it is necessary to install the dedicated app (Electronic Authentication My Number Sign).

- Applicants who possess a My Number Card (equipped with a valid electronic signature certificate) can request their own certificate.
- Identification using the My Number Card is required. Input of the card information entry assistance PIN (4-digit number) and the signature electronic certificate PIN (6 to 16 alphanumeric characters) is necessary.
- The certificate will be issued in writing and mailed, so you will also be responsible for the postage.
- Those living abroad cannot use the online application.

Online Application for Acceptance Certificate
- Only the family register application submitted to Inagi City can be applied for.
- Only the registrant of the family register can apply. (For example, the father cannot apply for the receipt certificate of the birth notification where the mother is the registrant.)

In addition to the certificate issuance fee, you will be responsible for the postal fee of 110 yen. (If the postal fee is insufficient due to weight overage, it will be sent as "shortage paid by the recipient.")

- Those eligible for support measures such as DV cannot apply online. (Please apply at the counter)
- Online applications do not qualify for fee exemptions (due to reasons such as receiving Public Assistance). Additionally, refunds after application cannot be accommodated.
- Cancellations are not possible after payment has been made. Please check your application details carefully before making a payment.
