Eligible Applicants
- The person themselves or someone from the same household
- Agent with a power of attorney
Notice: The power of attorney should be filled out by the delegator. (Download Application Form "Forms Related to Power of Attorney" page)
Required Items for Billing
- Identification documents of the applicants who came to the counter (For information on identification documents, please check the page on "Identification for Various Notifications and Requests.")
- Documents for confirming the authority of legal representatives such as guardianship (We may need to verify the contents of the documents.)

Points to Note When Requesting a Certificate of Residence Copy
- Regarding requests from Japanese residents, unless there are special requests, the registered domicile, head of household, relationship, name of the head of household, Certificate of Residence code, and personal number (My Number) will generally be omitted.
- Regarding requests from foreign residents, unless there are special requests, the relationship to the head of the household, the name of the head of the household, nationality/region, residency status, alias history, Certificate of Residence code, and personal number (My Number) will generally be omitted.
- In the case of a third-party request, supporting documents (related materials) corresponding to the reason for the request are required.
- The copies of the Certificate of Residence containing the residence code and personal number (My Number) requested by the agent will be sent by mail to the principal. Please note that they cannot be issued on the spot.

- If you have mistakenly written something, please rewrite it on a new form without correcting it.
