Note: If you lose your seal registration certificate (card), you cannot request a seal registration certificate even if you have your My Number Card. In that case, you will need to re-register your seal.

If an agent is applying, it is a condition that the registrant's address, name, and date of birth are correctly entered on the Application Form.
