Individual Consultation for Benefits (Information for Business Operators)
If you wish to introduce the following services, a prior individual consultation with the city is required.

- Home care services and living assistance for families living together
- Regarding the rental of welfare equipment for individuals with mild disabilities (limited to cases requiring city confirmation)
- Use of short-term residential care (short-term stay medical care) exceeding half of the certified validity period
- Other items that may require individual consultation

Submission and Consultation Deadline
- In the case of items 1, 2, 3, and 5, before the service implementation
- In the case of Content 4, before it is expected to exceed half of the certification validity period.
- If it is determined to be "appropriate for benefits" and you wish to continue using the service after the application period, please do so before the end of the application period.
- Note 1: Please submit your documents considering that it generally takes about 2 weeks for review after they are received.
- Note 2: If you are unable to submit before the deadline due to circumstances, please be sure to inform the benefits officer.
- Note 3: If it is determined that the service is "inappropriate for benefits" after its implementation, the portion of the service will not be eligible for nursing care benefits.
