The city is conducting care plan review meetings as part of the benefit optimization efforts.

Implementation Policy and Objectives

This is part of the optimization of benefits, but rather than a directive approach, it utilizes the 'Re-assessment Support Sheet' to conduct assessments (issue analysis) together with the city, chief care managers, and care managers. This encourages care managers to gain 'awareness' and jointly pursue what constitutes 'care management that contributes to independence,' aiming for its universalization and supporting the implementation of sound benefits. Additionally, it aims to improve the quality of care management by care managers.

Target of Implementation

Care managers belonging to the following facilities located within the city

  • Home Care Support Office
  • Small-scale multifunctional home care service office
  • Small-Scale Multifunctional Home Care Service

Implementation Method

  1. The city will request businesses to attend the study meeting. (Approximately 2 months before the scheduled implementation)
  2. The care support specialists at the business office will submit preliminary materials (preliminary submission forms, basic information sheets, re-assessment support sheets, home service plans) to the city. (Approximately one month before the planned implementation)
  3. We will hold a care plan review meeting and conduct an inspection of the care plan. (The meeting is generally held on the third Tuesday of every month in the afternoon)

Participants of the Discussion Meeting

  • Care managers and witnesses who submitted care plans, etc.
  • Chief Care Manager (Community General Support Center, In-Home Care Support Offices, etc.)
  • Long-Term Care Insurance Section, Senior Welfare Division
