Reissuance of Signboard Issuance Certificate
If you have lost your sign issuance certificate, you can apply for reissuance by mail without having to come to the counter. (No fees will be charged for reissuance.)
Those who wish to request reissuance by mail will need the following documents. Please be aware that reissuance may be delayed if there are any deficiencies or missing documents.
Required Documents for When the Owner Applies
- Copy of identification documents (driver's license, Health Insurance Card, My Number Card, etc.)
- Application Form for Reissuance of Light Motor Vehicle License Certificate (Please download the PDF file below, print it out, fill in the necessary information, and enclose it.)
- Reply envelope (limited to those with the recipient's address written and postage affixed)
Required Documents for Application by an Agent
If an agent is applying, in addition to the three necessary documents mentioned above, the following documents will be required.
- Power of Attorney (Required even for applications by relatives in the same household.)
