Notification of adoption

effective upon notification

• Municipal office of the permanent address of the adoptive parent or adopted child
• Municipal office where the adoptive parent or adopted child is located

Adoptive Parents and Adopted Children (If the adopted child is under the age of 15, the adopted child's legal representative)

1. Adoption notification (requires two adult witnesses)
2. Identity verification documents of the person who came to report
3. Individual number card (only for those who have the card and want to change their information. It also serves as an identity verification document.)
4. Basic Resident Registration Card (Only for those who have the card and want to change their information. It also serves as an identification document.)
5. When adopting a minor, a letter of permission from the family court (however, it is not necessary when adopting a direct descendant of oneself or one's spouse)
6. If an adoptive parent with a spouse or the person to be adopted adopts the child alone, the consent of the spouse is required.

Available date and time

From 8:30 a.m. to 5:00 p.m. on open days Remarks: On holidays, from 8:30 a.m. to 11:45 a.m. and from 1:00 p.m. to 5:00 p.m. increase.
Only "custody" is accepted on holiday open days and at the city hall night duty room.
Note: Sub-branch offices do not accept family register notifications.
