- Death certificate (doctor's certificate required)
Issuance of cremation permit
After receiving the death certificate, we will issue a cremation permit.
We will ask you which crematorium to use at the window, so please come to know.
Available date and time
From 8:30 a.m. to 5:00 p.m. on open days Remarks: On holidays, from 8:30 a.m. to 11:45 a.m. and from 1:00 p.m. to 5:00 p.m. increase.
Only "custody" is accepted on holiday open days and at the city hall night duty room.
Note: Sub-branch offices do not accept family register notifications.
