FY2024 Health Checkup

The campaign will run until Thursday, October 31, 2024.
We carry out medical examination for the latter-stage elderly medical system.
A ticket will be sent to those who are eligible.
How to see a doctor
- Please make a reservation directly with the medical institution conducting the medical examination. Note: Please tell us that you would like to make a reservation for a medical checkup for the latter-stage elderly.
- Please fill out in advance on the back side (questionnaire) of the medical checkup ticket for the latter-stage elderly.

What to bring when visiting a doctor
Please bring the following two items.
- Late-stage elderly medical checkup ticket
- Latter-stage elderly medical insurance card
