Application when making assistive devices, etc.

If you make corsets, casts, or other assistive devices under the doctor's instructions, you can apply for a refund for the portion other than the co-payment.

What you need to apply

1. Insurance card or certificate of eligibility
2. Statement of opinion (medical certificate) requiring assistive devices, certificate
3. Receipt
4. The transfer destination can be confirmed
5. Things that can be confirmed by My Number
