The system is operated by a "wide-area union" established on a prefecture-by-prefecture basis.
It is operated by the Tokyo Metropolitan Association for Medical Care for the Advanced Elderly, in which all municipalities in Tokyo (62 local governments) are members.
The regional union certifies insured persons (qualification), determines insurance premiums (imposition), pays medical expenses (benefits), and implements health services.
At the city hall, we will deliver insurance cards, accept various applications, and collect insurance premiums.
The financial resources of this system are 50% public funds, 40% support money from each insurer (National Health Insurance, social insurance, etc.) for people up to 74 years old, and 10% insurance premiums from members. .
