Application for high-cost nursing care combined medical expenses

The total amount of the co-payments of the medical care system for the elderly paid by the household in one year (from August 1st of each year to July 31st of the following year) and the amount paid by the user of long-term care insurance is the self-pay amount in the table below. If the amount exceeds the maximum burden, if you apply and it is approved, the excess amount will be refunded from the medical care system for the elderly and the long-term care insurance system.
Applicants will receive an application form from the Regional Union around March every year. If you are not sure whether this applies to you, please contact us.

  1. Application form sent from the Tokyo Metropolitan Association for Medical Care for the Advanced Elderly

  2. insurance card

  3. The transfer destination can be confirmed

  4. Documents that can verify the identity of the applicant (My number card, driver's license, etc.)

  5. Things that can be confirmed by My Number

  6. Self-pay certificate (only if necessary for those who have moved)

Note: If someone other than the applicant brings the application form, etc., please bring something that can verify the identity of the person who brings the application form.
