Application for funeral expenses

If a person dies while enrolled in medical insurance for the latter-stage elderly and a funeral is held, a subsidy of 50,000 yen will be provided to the person who performed the funeral.

What you need to apply

1. Funeral bill or receipt

2. The transfer destination can be confirmed

3. Documents that can verify the identity of the applicant (My number card, driver's license, etc.)

Note: If someone other than the applicant brings the application form, etc., please bring something that can verify the identity of the person who brings the application form.
