If you have lost your medical insurance card for the latter-stage elderly, etc., please fill in the application form (re-issuance application form for medical care for the latter-stage elderly) and submit it by mail or at the office so that it can be reissued. is.

If the insured person (the person himself/herself) is visiting the agency, please bring the following necessary items and come to the Health Insurance and Pension Section/Late-stage Elderly Medical Care Section at Counter No. 3 on the 1st floor of the city hall.

About application by mail

If you wish to apply and receive the certificate by mail, please download the form below, print it out, fill in the necessary information, and mail it to us. (Please contact us if you would like us to mail you the application form.) As soon as the application arrives at the city hall, we will create a certificate and send it to you by simple registered mail.

About application at city hall

Reissuance of insurance card

If you wish to apply and have the certificate issued at the city hall, please wait for the identification card of the insured person (the person in question). (Driver's license, my number card, passport, long-term care insurance card, medical insurance premium decision notice for the elderly aged 75 or over, cash card, etc.) In that case, two points will be checked.)

Note: If an agent visits the office, please bring something that can verify the identity of the agent.
In addition, if the procedure is performed by a proxy who lives with the insured person (the person in question), if the proxy's identity can be verified, the medical insurance card for the latter-stage elderly can be issued at the counter.
If the insured person (the insured person) and his/her representative living separately perform the procedure, the application form can be filled out and submitted, but the issuance of the medical insurance card for the elderly aged 75 or over can be sent by simple registered mail. becomes.

About reissuing other than insurance card

If you wish to have the maximum amount application/standard copayment reduction certificate, maximum amount application certificate, or specific illness treatment certificate reissued at the city hall, please wait for the insured person's (person's) ID. Hirao/Wakabadai Branch Offices can also fill out and submit application forms for reissuance of medical insurance cards for the elderly aged 75 or over.
