About changing the destination of nursing care insurance-related documents

If it is difficult to receive documents related to long-term care insurance at the insured person's address due to unavoidable circumstances and you wish to have them sent to an address other than the address listed on the insured person's resident card, please refer to the "Nursing Care Insurance Related Documents Sending Address". You can change the shipping address by submitting a "Setting Notification". If you wish to change or cancel a delivery address that has already been set, a similar notification is required.

Notes

  • The only mailing items whose destination can be changed using this destination setting notification are long-term care insurance-related documents.
  • You cannot change the delivery address to a nursing care facility, hospital, home care support office, nursing care service office, etc. In addition, please refrain from changing your residence for a short period of time (about one or two months) to a location that is not considered your home.
  • After changing the shipping address, if there is a change in the registered shipping address due to moving etc., a new notification is required.
  • After changing the delivery address, nursing care insurance documents will be sent to the new address unless you notify us of the change.
  • If you wish to change or cancel the registered delivery address in the "Nursing Care Insurance Related Document Delivery Address Setting Notification", you will be required to submit a new notification, which must be filled out with the previous applicant's signature.

★Identification documents: Current valid certificate issued by a government agency (My number card, driver's license, etc. Two items are required for health insurance cards, nursing care insurance cards, etc. that do not have a photo)
