If you are going to establish a designated work site, or if you have already established a designated work site and are changing the method of work or machinery, you are required to submit a notification of establishment (change) of the designated work site based on the Tokyo Metropolitan Ordinance on Ensuring the Health and Safety of Citizens (hereinafter referred to as the "Environmental Protection Ordinance").
Additionally, when establishing a designated work site, please confirm in advance whether it complies with other laws and regulations, such as the restrictions on industries and work site area by zoning as stipulated in the Building Standards Act and others.

Procedure for Installation

Documents Required for Notification

  1. "Notification of Designated Work Site Establishment (Change)" Form No. 16, Styles 1 and 2
  2. Forms 1 to 12 of No. 16 (varies by industry).
  3. Appendix List
  4. Guide Map (Map of the Area Within 50 Meters)
  5. Site and Building Layout Plan (Neighborhood Relationship Diagram)
  6. Floor Plan (Facility Layout)
  7. Elevation Drawing
  8. Equipment Structural Diagram (Machine Catalog, etc.)

In addition, depending on the type of designated workplace, there are required attachments.

How to Create a Notification Form

  • When creating, please write with a ballpoint pen or fountain pen if you are writing by hand.
  • Please prepare two copies of the notification form, one original and one copy. You may use a duplicate paper for the copy. The copy will be provided after the completion inspection.
  • Please refer to the example for instructions on filling out and creating the notification form.

Note: Regarding various notification documents, due to the abolition of seals, please correct any errors in the content by using double lines or similar methods.

Notes on Submission and Return of Notification Forms

  • Please contact the city representative in advance and have the person responsible or the one intending to set up the designated work site come to the city office.
  • Copies of the notification will generally be returned at a later date. If it is difficult for you to come to the office for the return, it will be sent by mail, so if you wish to have it mailed back, please attach a self-addressed envelope with the necessary postage when submitting the notification.
