About reissue of mark grant certificate
If you have lost the certificate of issuance of the mark, you can now apply for reissuance by mail even if you do not come to the window. (You will not be charged for reissuance.)
If you wish to have your certificate reissued by mail, the following documents are required. Please note that reissuance may be delayed if the documents are incomplete or insufficient.
Required documents when the owner himself/herself applies
- Copy of identity verification documents (driver's license, insurance card, My Number card, etc.)
- Light Vehicle Marking Certificate Reissuance Application Form (Please download the PDF file below, print it out, fill in the necessary information, and enclose it.)
- Self-addressed envelope (limited to those with the return address and stamps affixed)
Required documents when an agent applies
In addition to the three required documents above, the following documents are required in addition to the documents listed above if a proxy is applying.
- Power of Attorney (Required even if the application is made by a relative of the same household.)
