Regarding re-issuance of mark issuance certificate
If you have lost your sign issuance certificate, you can now apply for a reissue by mail without having to come to the counter. (There will be no charge for reissuance.)
If you wish to have your certificate reissued by mail, you will need the following documents: Please note that reissuance may be delayed if the documents are incomplete or missing.
Required documents if the owner himself/herself applies
- Copy of identity verification documents (driver's license, insurance card, My Number card, etc.)
- Light Vehicle License Certificate Reissue Application Form (Please download the PDF file below, print it out, fill in the necessary information and enclose it.)
- A return envelope (only one with the return address written on it and a stamp affixed)
Required documents if a representative applies
If a representative is applying, in addition to the three required documents listed above, the following additional documents will be required.
- A letter of attorney (required even if the application is made by a relative living in the same household)
