Change of My Number Card Design for Those Moving Abroad (Name Change)
If the name on the My Number Card of those moving abroad changes, a procedure for changing the card is required.
You can complete the procedure at your registered municipality.
Application Location (Registered Domicile in Inagi City)
Citizen Affairs Division of Inagi City Hall, Hirao Branch Office, Wakabadai Branch Office (Procedures can be completed on-site)
Note: If you apply at an overseas diplomatic mission, this will be information regarding the reissuance application for the My Number Card.
Required Documents (Permanent Residence in Inagi City)
Personal Application
My Number Card (PIN verification is required at the counter)
Application for Legal Representative (for those under 15 years old or adult wards) / Application by Spouse
- The individual's My Number Card (PIN verification is required at the counter)
- One official photo ID issued by a public office of a legal representative or spouse 1 point (such as a passport or My Number Card)
- Certificate of Registered Matters (for adults under guardianship)
Points to Note
- If you are unable to enter the 4-digit PIN, you will need to reset your PIN. For more details, please check 'My Number Card PIN Change/Reset for Those Moving Abroad'.
- The signature electronic certificate included in the My Number Card will become invalid if the name is changed. If a new one is to be issued, the individual must come to the office to complete the procedure (verification of two types of PINs: a 4-digit PIN and the signature electronic certificate PIN is required).
